Rehabilitation
The team meets regularly once a week, to discuss management goals, while the Nurses, Physios and OTs get together on a daily basis to discuss treatment plans in more detail.
A few people, who have had only a mild stroke, may return straight home, where their care is provided by the Community team, assisted by the MDT in St Magnus Day Hospital.
Others, who are less fortunate and have had a severe stroke, or have other medical conditions that make it difficult for them to participate in an active rehabilitation program, are looked after in St Ninian Ward, where the same team is involved, but on a less intensive basis, allowing them to reach their maximum potential over a more leisurely timescale.
Rehabilitation is provided in a range of settings including :
-
-
Step Down Care -Care of the Elderly Ward
-
-
GP Support
-
Early Supported Discharge home based therapy.
-
Domiciliary Services
